Located within Mercy Medical Center St. Mary's, the Transitional Care Unit is a hospital-based skilled nursing facility providing sub acute care to facilitate a patient's transition from acute care to the appropriate care environment. The expected length of stay for patients on the Transitional Care Unit is short term – 30 days or less.
Is transitional care right for me?
Ask your physician. Based on his/her medical findings and your needs, transitional care may be appropriate.
For Medicare recipients, patients must have a three-day medically necessary, qualifying hospital stay.
Physician orders must prescribe a planned regimen of total patient care based on a patient's immediate and long-term care needs.
Patients must require skilled nursing care daily or rehabilitation services (physical, occupational, or speech therapy) five days a week.
What services are available in transitional care?
24-hour nursing service that provides continuous and comprehensive care under the direction of a physician.
Rehabilitation on services to improve the patient's level of functioning.
Physical Therapy
Occupational Therapy
Speech and Language Therapy
Appropriate recreational, social, and spiritual activities based on a patient's interests and needs.
Recommendations and assistance with development of discharge plans that promote continuity of care.
