Our programs provide social support to members of our communities that suffer from chronic conditions, like asthma, diabetes and hypertension (high blood pressure) and lack access to care. We want to help individuals focus on living rather than their condition. Our programs strive to reduce emergency room visits re-admission rates, increase primary care physician referrals and find long-term solutions for access to medication.

Support is provided through home intervention efforts, and works to help patients with the following:

  • Coping skills to prevent re-hospitalization
  • Medication management
  • House and transportation resources
  • Develop family support systems
