St. Rita’s Health Management Group is an approach to patient care that emphasizes coordinated, comprehensive care for those coping with chronic conditions such as asthma, heart failure, diabetes, obesity or any condition requiring use blood thinners. Careful management of these conditions is essential for patients to reduce their risk of complications and potential hospitalizations.
It is important for the patient to play an active role in the management of their health conditions. St. Rita’s professionals work with physicians and their patients to develop an individualized treatment plan designed to keep their disease under control and to prevent frequent readmissions to the hospital. By working closely with the patient, the Health Management Group can help improve understanding of their health conditions, reduce complications and help keep patients healthy in between their physician appointments.
St. Rita’s Health Management Group offers programs in:

The Health Management Group is staffed by clinical pharmacists, registered nurses, clinical dietitians, respiratory therapists, and medical assistants. A social worker and exercise physiologists are available, if needed. In all clinics, we have three referrals options. These include education only, education and assessment with the referring provider being called for orders, and education and assessment with the clinic managing the medications. There are further options for the provider in regards to the specific medications that we manage. We have on-site laboratory services as well as a continuous glucose monitor, ECG, thoracic electrical bioimpedance, spirometry, pulmonary stress testing, and ABI.
Education is integral with all visits but also separated into weekly group sessions. Training for insulin pumps, continuous glucose monitors, POC INR meters, and glucose meters is provided. Coordination of care with providers is integral and can include emergent conditions and procedures. All progress notes are faxed to the referring provider by the end of the day. We utilize the institution's EMR at varying amounts for all clinics. Finally, we provide community screenings for a variety of disease states and speak at community groups, when invited.
St. Rita’s Health Management Group is open Monday through Friday from 8 am to 6:00 pm. The clinic is located at 770 W. High St., Suite 450. Patients may use the parking lot located off of North Street or the parking garage off Market St
