The law requires that hospitals admitting you ask if you have Advance Directives. These include:
  • Durable Power of Attorney for health care, which involves naming someone to make health care decisions for you if you are not able to do so.
  • Living Will, which states the extent of your willingness to use life-support methods to lengthen your life when there is no hope of recovering.
  • Organ Donation, which asks you to indicate your wishes about donating your organs.
These issues can be complex and patients and families may not always be certain how they wish to fill out the Advance Directive forms. To help you make the decisions that are right for you and your family, our chaplain staff is available to explain these issues to you and to help you complete the paperwork free of charge.
