At Oakwood Village, we’ve created a place where individuals who need short-term inpatient rehabilitation can focus on building the strength, skills and confidence to return home. Patients staying in our private Transitions suites receive state-of-the-art physical, occupational and speech therapy, provided in a comfortable, caring environment.

Our 30 all-private suites also create the peaceful environment patients need to recover. Every suite features:

  • Electric 42-inch-wide therapeutic adjustable beds

  • One-of-a-kind view
  • Selective  menu options  that can be delivered to the room or served in our spacious dining room

Transitions suites also are part of Oakwood Village’s beautiful 72-acre campus, which has safe walking paths, woods, prairies, courtyards and a pond. 


Support for caregivers

To support our residents’ families, friends and referring doctors, as well as visiting healthcare staff, Transitions includes convenient services and amenities:

  • A café with free soda and coffee
  • A business center with desktop computers and Internet
  • Around-the-clock admissions
  • Dining area with a fireplace and a courtyard patio for enjoying meals


Individualized therapy

We offer advanced physical, occupational and speech therapy for people recovering from a range of health problems and procedures, including:

  • Heart surgery and cardiac recovery services
  • Orthopedic injury or surgery 
  • Stroke and neurologic conditions
  • Pulmonary Care treating lung conditions
  • Wounds and complex medical treatments 

Our goal is to provide the compassionate support each patient needs to safely return home and function at the highest level possible.  


High-quality team care


To offer the best care for patients’ immediate needs and long-term rehabilitation, we use a team approach 
that involves:

  • The referring doctor or specialist, who receives regular progress updates and rehabilitation outcomes
  • A medical director who is on-site five days a week
  • Around-the-clock nursing care
  • Physical, occupational and speech-language therapists from RehabCare, a nationally recognized leader in rehabilitation


State-of-the-art rehabilitation 


Transitions patients receive the most advanced therapy available. In addition to weights, rebounders, recumbent bicycles, treadmills and standing tables, our new 2,800-square-foot gym offers:

  • A car transfer simulator, where patients can practice getting into and out of a car in a safe, comfortable indoor setting
  • A sky lift for patients with limited mobility
  • A putting green for improving balance and practicing real-life activities
  • A range of therapeutic technologies
  • Environmental home assessments and modifications

 
Continuing care

The range of services at Oakwood Village makes it easy for patients to transition to a different level of care. Before patients return home, a rehabilitation expert helps find solutions for factors that might limit a patient’s mobility or independence in and around their home. As a continuing care retirement community, the campus also offers:

  • One-on-one therapy available seven days a week
  • An outpatient treatment program for patients to continue improving their skills and independence
  • Independent living cottages, apartments, assisted-living, assisted-living memory care and  long term care skilled-nursing housing options
