Our staff of specialists has experience in adult and pediatric neurovascular surgery and uses minimally-invasive techniques to treat neurological diagnoses such as aneurysms, strokes and other vascular injuries.
Our neurologistsAhmed Arshad, MD
Dr. Arshad is board-certified in neurology and neurophysiology and brings expert skill, training and education to the treatment of neurological issues. Dr. Arshad received his medical degree from King Edward Medical College in Lahore, Pakistan, and completed a residency in neurology at The University of Oklahoma in Oklahoma City, OK. Dr. Arshad completed a neurophysiology fellowship from St. Louis University Hospital in St. Louis, MO. A member of Mercy Neurology Associates, Dr. Arshad is an expert in adult neurology including memory loss, EMG/neurophysiology, and Botox ® use for medical reasons, including headaches.
E. Tomas Calderon, MD, Department Chairman
Dr. Calderon completed a neurology fellowship at Georgetown University Hospital in Washington, D.C. after earning his medical degree from The Ohio State University. Board-certified in neurology and sleep medicine, Dr. Calderon is fluent in Spanish as well as English. His special interests include sleep disorders, restless leg syndrome and general neurology including strokes, seizures, headaches and back pain.
Subrahamanyam (Sam) Chodisetty, MD
Dr. Chodisetty is a board-certified neurologist who is fellowship trained in stroke and clinical neurophysiology.
Dr. Chodisetty received his medical degree from Andhra University in Visakhapatnam, India. He completed residencies in neurology from the Medical College of VIriginia Hospitals in Richmond, VA, and in internal medicine from Bronx-Lebanon Hospital Center in Bronx, NY. Dr. Chodisetty attended a stroke fellowship at Dent Neurologic Institute at Millard Fillmore Hospital in Buffalo, NY, and completed a fellowship in clinical neurophysiology from the Medical College of Virginia Hospitals.
With several decades of experience, Dr. Chodisetty provides expert care to patients with a special interest in adult neurology, electromyography (EMG), stroke, seizures, and Multiple Sclerosis.
Peter Zangara, MD
Dr. Zangara is board-certified in neurology and has more than 35 years’ experience treating neurological conditions. He is also board certified in neurophysiology (EEG and evoked potentials) and electrodiagnostic medicine (EMG). Dr. Zangara completed his residency at Baylor College of Medicine in Houston, TX after receiving his medical degree from New Jersey Medical School in Newark. He is interested in treating patients with epilepsy, multiple sclerosis and neuromuscular disease.
Our pediatric neurologistAteeq Haseeb, MD, MBA, CPE, FASNM
Dr. Haseeb is a pediatric neurologist and epileptologist at Mercy Children’s Hospital. Dr. Haseeb serves both children and adults throughout the region with neurological conditions including headaches, ADHD, epilepsy, developmental delays, spasticity and other neurological disorders.
Dr. Haseeb obtained his medical degree from the NTR University of Health Services in India. His residencies were in pediatrics at Stroger Hospital of Cook County, Chicago, IL and in adult and pediatric neurology at Wayne State University, Detroit, MI. He also completed additional fellowship training in Clinical Neurophysiology/Epilepsy program at the Detroit Medical Center, Detroit, MI. Dr. Haseeb has also earned a Master’s of Business Administration degree at the University of Michigan, Ann Arbor, MI.
Dr. Haseeb possesses multiple board certifications in the fields of neurology and pediatric neurology, clinical neurophysiology, Epilepsy and Intraoperative monitoring. He is a member of American Academy of Neurology, American Telemedicine Association, American Epilepsy Society as well as American Cinical Neurophysiology Society.
Dr Haseeb is dedicated in taking care of children with special needs and participates in the neurological care of children from several residential nursing homes in the Toledo region. He is also a board member for the Family Child Abuse Prevention Center (FCAPC) in Toledo and actively participates in local programs that seek to end abuse against women and children. He volunteers in activities and fundraisers designed to support local community events and contributes to projects aimed at creating awareness for leading debilitating conditions like stroke, heart disease as well as epilepsy. His dedication to his job ensures that he is always available to provide confident, and experienced medical care to the community.
Our neuro endovascular surgeonsEugene Lin, MD
Dr. Lin is a stroke and endovascular neurosurgery/neurointerventional clinical specialist, who is board certified in neurology and vascular neurology. Dr. Lin received his medical degree from Northwestern University, Feinberg School of Medicine, in Chicago, and completed a preliminary residency at St. Francis Hospital, also in in Evanston, IL. He also completed a neurology residency and vascular neurology fellowship at the University of Massachusetts Medical School in Worcester, MA. Dr. Lin completed his Interventional Neuroradiology fellowship at Baptist Cardiac and Vascular Institute.
Dr. Lin provides increased access and awareness to the specialized care, diagnosis, and treatment of complex neurovascular syndromes ranging from aneurysms to hemorrhagic and ischemic strokes. Dr. Lin’s expertise includes minimally invasive management of intra/extra cranial vasculopathies malformations, complex cerebral aneurysms, minimally invasive complex aneurysm emobilization and vessel reconstruction and minimally invasive repair of traumatic head and neck vascular injuries.
As a leading member of the team who brought the Mercy Life Flight Network Mobile Stroke Unit (MSU) to Northwest Ohio, Dr. Lin is called upon to complete expert evaluations via telepresence so as to begin treatment of the patients with isischemic and hemorrhagic strokes. The Mercy MSU is only the fourth nationwide and is designed as an emergency room on wheels to provide a means to better deliver appropriate stroke care when it is needed most.
Sam Zaidat, MD, MS, FAHA, FAAN, Mercy Health Neuroscience & Stroke Medical Director
Dr. Zaidat is a stroke and endovascular neurosurgery/neurointerventional clinical specialist, who is certified by the American Board of Neurology, American Board of Spinal Cord Medicine, American Board of Vascular Neurology, and United Council of Neurological Specialties (UCNS) in Neurocritical Care.
Dr. Zaidat received his medical degree from Jordan University Medical School in Amman, Jordan and completed neurology training at Case Western Reserve University in Cleveland. He also completed a Neurocritical Care and Stroke Fellowship Case Western Reserve University and was a clinical associate and fellow in the Department of Radiology, Neurointerventional Program at Duke University and Medical Center in Durham, NC.
Prior to joining Mercy Health, Dr. Zaidat was a professor of Neurology, Neurosurgery, and Radiology and the Director of Comprehensive Stroke Center and Neurointervention at the Medical College of Wisconsin, Milwaukee, WI.
Dr. Zaidat brings to Toledo and Mercy Health a wealth of experience in the field of stroke, neurointervention and cerebrovascular diseases. He is noted as a world leader in the field of stroke and neurointervention and has written more than 180 peer reviewed articles, participated at the principle investigators in national and international brain aneurysms and ischemic stroke clinical trials, and served as co-leader of the endovascular committee of the National Institutes of Health StrokeNet consortium.
Srinivas C.T. Chakravarthi, MD, Chairman, Neurosurgery
Dr. Chakravarthi specializes in complex neurological procedures such as brain tumors, aneurysms, spine and pain relief procedures.
Dr. Chakravarthi received his medical degree, and then completed a General Surgery fellowship, at Banaras Hindu University in India. He received further general surgery training at the University of Ottawa in Ontario, Canada. He went on to be the chief neurosurgery resident at G.B. Pant Hospital in New Delhi. Dr. Chakravarthi completed neurosurgery fellowship training at the University of Alberta, in Edmonton, Canada. While at the University of Alberta he worked as a teaching fellow in neurosurgery and also served as the associate director of Spinal Injury Rehabilitation. He is certified in neurosurgery by the Royal College of Physicians and Surgeons of Canada.
Prior to arriving at Mercy, Dr. Chakravarthi served as a neurosurgeon, including in leadership positions, in Hotel-Dieu Grace Healthcare in Windsor, Ontario as well as Bay Regional Medical Center in Bay City, Mich.
Dr. Chakravarthi performs a complete range of spinal surgical procedures, including tumors, vascular malformations, and minimally invasive as well as complete spinal fixation, with both anterior and posterior approaches. He also specializes in a complete range of brain surgeries, including aneurysms, anterior venous malformations, complex skull based surgeries, and pituitary surgeries. Dr. Chakravarthi also performs pain relief procedures such as spinal stimulation, pain pumps, and peripheral nerve surgeries.
Rathul Raychaudhuri, MD
Dr. Raychaudhuri provides expertise for a comprehensive care approach for proven neurological outcomes. Dr. Raychaudhuri received his Bachelor of Arts in Neuroscience from Johns Hopkins University and medical degree from Northwestern University in Evanston, IL. He completed a neurosurgery residency at the University of Missouri – Columbia, where he served as chief resident. Dr. Raychaudhuri specializes in general neurosurgery, cerebrovascular/aneurysm care, tumor treatment, trauma and spine surgery.
Zubair Ahammad, DO
Dr. Ahammad received his degree in Osteopathic Medicine from Michigan State University College of Osteopathic Medicine. He completed his Neurosurgery Residency at Grant Medical Center in Columbus, Ohio. Dr. Ahammad is a member of the American Osteopathic Association and the American College of Osteopathic Surgeons. Clinical interests include: peripheral nerve, neuro-oncology, general neurosurgery and spinal surgery.
Our physical rehabilitation specialistsDominique Lash, MD
Dr. Lash, is a board certified physician in Physical Medicine and Rehabilitation. She is passionate about helping patients deal with functional setbacks that may interfere with daily living. Dr. Lash works with patients to improve their maximal functional ability and quality of life.
Dr. Lash earned a bachelor’s of science from University of Michigan, in Ann Arbor, MI and earned her medical degree from the University of Illinois College of Medicine at Chicago. She completed post-doctoral training in Internal Medicine at Cook County Hospital and in Physical Medicine and Rehabilitation at Rush-Presbyterian St. Luke’s Medical Center in Chicago.
Prior to joining Mercy, Dr. Lash spent 12 years as an attending physician specializing in Physical Medicine and Rehabilitation in facilities in Chicago and Sylvania.
Dr. Lash specializes in working with patients with physical impairments and disabilities including back and neck pain, musculoskeletal disorders, osteoarthritis and osteoporosis as well as with patients rehabilitating as a result of a stroke.
Sanjay Shah, MD, Medical Director, Physical Medicine and Rehabilitation Services at Mercy Health - St. Vincent Medical Center and Mercy Health - St. Charles Hospital
Dr. Shah is board certified in Physical Medicine and Rehabilitation, Disability Analysis and Electrodiagnostic Medicine. Since 1999, Dr. Shah has helped patients in the Toledo area work through physical setbacks including stroke to get them back to living the life they want.
Dr. Shah is a graduate of the Pennsylvania State University College of Science and earned his Medical Degree from The Pennsylvania State University College of Medicine. He completed a Physical Medicine and Rehabilitation residency at Temple University Hospital, Philadelphia, PA.
