What is a patellar tendon tear? 

A patellar tendon tear is an injury where the tendon connecting the kneecap (patella) to the shinbone (tibia) is partially or completely torn. This tendon is essential for straightening the knee and supporting activities like walking, jumping, and climbing stairs.

Causes of a patellar tendon tear 

Patellar tendon tears often occur during sudden, forceful movements like jumping or landing awkwardly. They can also result from direct trauma to the knee or from chronic tendonitis that weakens the tendon over time. 

Risk factors for a patellar tendon tear 

Athletes are particularly vulnerable to tearing their patella tendons since they put more strain on it. Other risk factors include: 

  • Participation in sports that involve jumping (like basketball or volleyball) 
  • Chronic patellar tendonitis (known as "jumper’s knee") 
  • Previous knee injuries or surgeries 
  • Conditions like rheumatoid arthritis or diabetes 
  • Use of corticosteroids or fluoroquinolone antibiotics 

Symptoms of a patellar tendon tear 

Although it sounds like you’d immediately know you tore a tendon, common symptoms may be less noticeable and can include: 

  • Sudden pain in the front of the knee 
  • A popping or tearing sensation at the time of injury 
  • Inability to straighten the leg or support weight 
  • Swelling and bruising around the knee 
  • A gap or indentation just below the kneecap 

Diagnosing a patellar tendon tear 

Diagnosis begins with a physical exam by your doctor to assess the knee’s movement and strength. Imaging tests like X-rays can show if the kneecap has moved out of position. MRI is used to confirm the extent of the tendon tear. 

Treatment for a patellar tendon tear 

Partial tears may be treated with a combination of rest, a knee brace and physical therapy. Complete tears usually require surgery followed by a structured rehab program. Recovery can take several months and includes regaining strength and flexibility through physical therapy.
