MyChart- Your secure, online health connection

Your health is important to you around the clock - not just during office hours. That's why we offer MyChart. MyChart gives you the ability to easily schedule, change or cancel appointments any time of day. You can also use MyChart to start an Evisit, view your medical history or test results, ask your doctor a question, request a prescription refill and more.

New MyChart users

To begin using MyChart, enter your MyChart activation code on the MyChart registration page. This code is 10 digits and would have been provided at a recent visit.

If you don't have an activation code or need to request a new one, call the MyChart Help Desk at 844-552-4278.

Existing MyChart users

Login to MyChart to instantly access your medical information.

NEW: Schedule an appointment through MyChart

You can now schedule or request appointments with your established Mercy Health care providers through MyChart. Simply login to MyChart, then select "Visits" and "Schedule or request an appointment." Choose a type of visit, then select a provider and practice location to see current available appointments. If you do not see an open time slot that fits your needs or you'd like to schedule with your provider's office staff, you can request an appointment instead.

Requesting an appointment allows you to select a provider, the reason for your visit, your availability and any other information you'd like to include in a message to your provider's office. You may receive a message reply through MyChart or a phone call from the office to schedule an appointment.

You can also schedule an Evisit through MyChart, which gives you quick, convenient access to primary care providers who can diagnose non-urgent medical conditions without the need for an office visit.

Download the MyChart app

Take MyChart with you wherever you go by downloading the MyChart Mobile app for Apple ® or Android ™. Once you've downloaded the app simply select your health care provider, and enter your existing MyChart username and password.
