Located in Toledo, Ohio, the Mercy Health PGY2 Ambulatory Care Pharmacy Residency is a one-year training program that gives residents the opportunity to develop clinical expertise through medication therapy management (MTM) within the patient centered medical home (PCMH). The PCMH model provides care that is structured, delivered, and coordinated around the specific needs of each patient. By maximizing medication benefits the pharmacist is a part of the healthcare team working to improve outcomes related to chronic diseases. Sites for patient visits will include primary care provider offices, pharmacies, hospitals, and patient homes. The resident will be  involved with Diabetes Education group sessions using the Diabetes Conversational Maps. 

Throughout the year the resident will develop collaborative practice agreements, precept student activities, complete a research project, participate with committees, manage formularies, and learn about Lean Six Sigma principles.   

This residency program is accredited by the American Society of Health-System Pharmacists (ASHP).
