Mercy maintains adult medical records for 10 years from the date of service. Records for Minors are maintained until Age 20, but at least for 10 years.
You can request a copy of your medical records, or the medical records of someone you have legal representation over, by filling out the form below:

Records are kept off site so please allow time for processing. A picture ID is required when picking up medical records. If you select to have your medical records mailed to your home or to another location, a copy of your photo ID must be included with the mailed or faxed request.

Compassionate care is a top priority at Mercy Health – if necessary visitors may be asked to step out of the room so our caregivers can provide that care or limit their stay based on the patient’s condition.

Sign and foreign language interpreters provide hearing impaired and Limited English Proficient (LEP) patients interpreting services for medical treatment and care at no charge.
For deaf and hard of hearing patients, we offer TTY technology, light signalers and closed captioning.
For LEP patients, Spanish channels are available with the option of Spanish captioning. We also provide dual-handset phones for ease of communication with providers.
