Take control of your with with our vital sign monitoring program

Your doctor at Mercy Health would like you to take part in our vital sign monitoring program, which helps us make sure we’re giving the best care possible. It can also help keep you from being readmitted to the hospital. The program works by sending daily vital sign readings to a secure database from a cellular device. A nurse uses this information to find early warning signs to get you the care you need before a trip to the emergency room or doctor’s office is needed.

What you need to know:

• This program is run by Mercy Home Health Care.
Installation and training will be provided by Assured
Medical Alliance, who will call you to arrange an
installation date. They will set up the monitor, test it,
and train you on how it works. Installation and training
usually takes about 15-20 minutes.

• You will need to place the scale on a hard surface
(about a two foot by two foot area). The blood
pressure cuff and pulse oximeter can be in a different
room (like your living room).

• Take your vital sign readings daily at the same time.
It only takes a few minutes and you won’t need to be
hooked up to the monitor all the time.

• If your vital signs are higher or lower than normal, a
Mercy Health nurse will call to discuss your condition
and coordinate any necessary changes with your
doctor at Mercy.

• The monitor will be in your home for 30-60 days.
Assured Medical Alliance will remove the monitor
afterwards.
