Mercy Health Acquires New Care Locations in Fairborn
GREENE COUNTY, OH (December 1, 2025) – As part of Mercy Health – Springfield’s continued commitment to expanding access and making health care easier for patients, the health system is proud to announce the acquisition of two new care locations.
Beginning December 2nd, Mercy Health - Occupational Health Services, Fairborn Citran will open, allowing the health system to broaden its ability to meet the needs of local employers and their workforces. A day later on December 3rd, Mercy Health will welcome new patients to Mercy Health – Fairborn Family Medicine, formerly Western Medicine, located at 7774 Dayton Springfield Road in Fairborn.
The primary care practice includes Dr. Jacob T. Dean and three advanced practice clinicians – Jennifer M. Dyer, CNP, Victoria Hase, PA-C and Trent Nourse, PA-C – all of whom will join Mercy Health and continue offering high-quality, compassionate care. The team will offer preventative care, chronic disease management and treatment of immediate needs, with onsite services including X-ray and lab capabilities.
“These additions increase our ability to serve patients and employers across the southern Miami Valley,” explained Shane Hardy, vice president of Mercy Health’s medical group operations. “It is great news for the community to have more physician presence and support as well as more access points to increase the community’s connection to our continuum of care.”
Over the last several weeks, Mercy Health has collaborated closely with incoming physicians and associates to ensure a seamless transition for both locations.
