On the day of your procedure, please bring your insurance cards and photo ID with you for verification. If you have any questions, please contact our Customer Service Team. The Jewish Hospital — Mercy Health offers a complimentary concierge service, "Best Upon Request," for those admitted to the inpatient units and their guests. Our goal is to make a stay in the facility as comfortable and stress-free as possible. The service provides assistance with everything from charging cell phones to picking up prescriptions, coordinating pet sitters, or fueling up a car before discharge. There is no charge for the concierge service; users only pay for the cost of goods and services. For details, call the concierge at 513-745-1680 or ask your nurse to contact us on your behalf. The service is available Monday–Friday, 8:00 a.m.–4:30 p.m.
Security is available 24 hours a day and can be contacted through the hospital operator. Just dial 0 from any hospital phone.
If you lose something at the hospital, please call Security at 513-686-5685 so they can check the ‘Lost and Found’ supply.
You can request a copy of your medical records, or the medical records of someone you have legal representation over in addition to authorizing records to be sent to another party by completing the form below:

When will I receive a copy of my medical record?
Please allow 30 business days for us to process your request. In some cases, the information you need may be stored at one of our off-site storage locations so additional time may be required to process your request. Please be sure to fill out the authorization form accurately. Inaccurate information on the form may cause delays in providing you with the information you requested.
If your request requires a fee, you will be notified of the fee before the record is sent.
