How to Apply for PGY1 Pharmacy Residency Program

Application Qualifications

In order to be considered for this residency, the applicant must:

  1. Possess a strong interest in clinical pharmacy practice
  2. Demonstrate a desire to teach
  3. Be eligible for licensure in the state of Ohio
  4. Participate in an on-site interview 
  5. Complete an ACPE-accredited Pharm.D. Degree program
  6. Participate in the ASHP Resident Matching Program
  7. Be able to start July 1

Application Materials 

Before the January 5th deadline, the applicant must submit the following application materials via the Pharmacy Online Residency Centralized Application Service. You must include:

  • Letter of intent
  • Current curriculum vitae
  • Academic transcripts
  • Three references on standardized template

How to Apply for PGY2 Critical Care Pharmacy Residency Program

Application Qualifications

  • Possess a strong interest in critical care medicine clinical pharmacy practice
  • Demonstrate a desire to teach
  • Be eligible for licensure in the state of Ohio** Please note we are not able to sponsor visas for pharmacy residents
  • Participate in an on-site interview
  • Complete an ACPE-accredited Pharm.D. Degree program
  • Be enrolled in or previously completed an ASHP-accredited PGY1 Pharmacy Residency
  • Participate in the ASHP Resident Matching Program
  • Be able to start July 1
